Mental Health Assessment

The Child Center offers comprehensive Mental Health Assessments for children and adults who are experiencing difficulties at home, school or in the workplace that include but are not limited to:

• Family conflict
• School failure
• Disciplinary issues
• Foster care placement
• Self-harming behaviors
• Emerging mental health concerns                                                                                            • Persistent Mental Health Concerns

Mental Health Assessments are completed by Qualified Mental Health Practitioners (QMHP) and last approximately 90 minutes. During assessment, information gathered will include:

• Identifying information
• Presenting problems
• Precipitating events
• Developmental history
• Family composition
• Family history
• School functioning
• Social functioning

In addition, screenings for the following will occur:

Substance abuse screening
• Trauma screening
• Medical health screening
• Risk of harm screening                                                                                                             
• Diagnostic screening

 

Upon completion of assessment, clients may receive:

• Persistent mental health concerns
• Mental health diagnosis 
• Service plan with treatment goals and treatment modalities
• Recommendations for level of care

Clients seeking assessment can usually be seen within a week, although during busy times it may take longer due to high demand. The Child Center intake team will work with families to schedule assessments that accommodate their circumstances and level of urgency. In some cases, assessments can be scheduled after normal working hours, and possibly on the same day if there is a crisis level of urgency.

Referrals or requests for assessments in Lane County can be made by contacting The Child Center Intake Office at (541) 726-1465.

Central Oregon: Point of access for requesting mental health assessments is through our Bend office at (541) 728-0862 or through Deschutes County Mental Health at (541) 322-7500,where Child Center services may be requested.
